If you have an HMO insurance, please be sure to have your Primary Care Provider send us a referral to avoid delay in your care. We cannot see you until a referral is on file.
1st - Please fill out the Medication List and Medical Records Release Forms and email them back to info@beatscardiology.com. If you have trouble, then bring in the filled paper copies to your appointment. The signature line works in Adobe Reader, which there's a free version that you can download here: https://www.adobe.com/acrobat/pdf-reader.html.
Go to our patient portal (link - Click Here ) and set up an account. On the portal, please fill out your information under "My Account" and the "Questionnaires" medical history.
If you have all of the same info already in a form of your own, you can provide your information form instead of this form. Please email it as above.
